The Health and Social Care Board is consulting on the proposed permanent relocation of GP Out of Hours from Moneymore to Mid Ulster Hospital, Magherafelt.

It is important that we engage with those who use GP Out of Hours services in the Mid Ulster area to find out their views on the proposed permanent relocation. We are seeking views on the proposed relocation and would be very interested in hearing from you.

Further information about the consultation is provided below.  You can give us your views on this proposal by either completing the Online Response Questionnaire or by completing and posting the MS Word Response Questionnaire. Both Response Questionnaires are included in the consultation documentation below.
