Integrated Care

The concept of integrated care is about promoting ‘joined-up’ or partnership working among health-care practitioners:

A smoother and more efficient ‘patient journey’ in health care is achieved when all practitioners work closely and cohesively on behalf of patients to identify and deliver improved services to patients.

The HSCB’s Directorate of Integrated Care is responsible for commissioning and managing General Medical, Dental, Ophthalmic and Pharmacy services. It is also responsible for the development of Integrated Care Partnerships and the Primary Care Infrastructure Development Programme.
